Physicians and Clinics - Due to overwhelming demand for services, a new restrictive referral policy was implemented effective 12/9/2024. Very few referrals received on or after 12/9/2024 are being accepted. Referrals FAXed to 844-454-8351. Include a clear referral question with recent records along with the usual demographic information.
NEW Patients - Physician Referral is REQUIRED. No self referrals are accepted. Also see comment above regarding current restrictive NEW referral process

Payment Methods - for patient responsible amounts we accept cash, checks and major credit cards. Payments can be made on the Patient Portal.
